At Core Health we strive to make every patient feel welcome and comfortable during their first visit. We have put together a brief summary of what you can expect. Please take a moment and make sure you have all of the information you will need for your first visit with us. We look forward to seeing you.
- Scheduling your Appointment.
If you haven't done so already, please call the office at 858.272.0074 to schedule your first appointment. One of our staff members will advise you on required insurance and patient information, and answer any specific questions you may about your first visit.
- Your Appointment.
In order to ensure all patients are seen at the scheduled time, we would like you to arrive approximately 15 minutes before your appointment. We reserve the right to reschedule any patient who arrives more than 15 minutes late.
- Patient Information.
Please contact our office so we can email or fax you our intake forms to complete prior to your visit. Be sure to bring your insurance cards, patient profile intake forms and payment for your first visit. Payment is due at the time of service.
- Patient Consultation.

Your doctor will meet with you to perform a thorough examination, complete your first therapy treatment, and determine your ongoing treatment plan as well as discuss your post treatment care instructions with you.
- Next Visit.
After your first visit is complete, you can make additional treatment appointments as specified by your treatment plan.
